Drug addiction causes sufferers to experience physical and psychological dependency on illicit, mind-altering substances. Habitual drug use causes changes in the structure and operation of the brain that deepen and reinforce drug addiction, to the point where a desire to stop using drugs is not enough to make it happen. For example, drinking as a way of coping with difficulties or stress, instead of confronting the sources of those difficulties or stressors, is an early indication that someone is relying too heavily on alcohol. The first step in recovery is deciding if you have a problem. This can be difficult, because your addicted-self will try hard to convince you that you don't have a problem. This is where a trained professional can gently help. They can keep you from tricking yourself and prevent you from slipping back into denial. They are trained to look for signs of trouble.
If someone you love has a drinking problem, you may be struggling with how to help them and how to avoid being overwhelmed. You are probably feeling a combination of anger, fear, shame, and sometimes even guilt. At times, it may seem easier to ignore the problem. SMART Recovery was founded by Joe Gerstein in 1994 by basing REBT as a foundation. It gives importance to the human agency in overcoming addiction and focuses on self-empowerment and self-reliance.[32] It does not subscribe to disease theory and powerlessness.[33] The group meetings involve open discussions, questioning decisions and forming corrective measures through assertive exercises. Inpatient residential rehab involves an extended time period for treatment, regardless of the substance. Programs typically last 30–45 days, or longer, depending on each client’s needs. Clients are required to stay at the facility for the entirety of the program, including overnight. For the average person who does not suffer from alcohol addiction, a drink every now and again is both normal and non-problematic. Made from a mixture of baking soda and powder cocaine, crack is a version of cocaine—but at a lower purity level. The key difference is that crack is smoked. This method of ingestion allows the drug to seep into lung tissues, producing a completely different result. Smoking crack causes the high to be much faster and more intense than the high traditionally felt from powder cocaine. The first step in treatment is brief intervention. The physician states unequivocally that the patient has a problem with alcohol and emphasizes that this determination stems from the consequences of alcohol in that patient's life, not from the quantity of alcohol consumed. Emphasizing the effects on family, friends, and occupation, as well as any physical manifestations, is important. The patient's response determines the physician's next step. If the patient denies the problem, recommending joining AA will not work. Involving the family and/or suggesting a trial of abstinence is useful, and, importantly, the physician should follow up with the patient in a few weeks. The patient might be angry initially and storm out of the office, but then the patient might recall the physician's warning months or years later and stop drinking. For patients who recognize a problem and will consider referral, the cheapest (free) and most accessible option is AA.

An influential cognitive-behavioral approach to addiction recovery and therapy has been Alan Marlatt's (1985) Relapse Prevention approach.[40] Marlatt describes four psycho-social processes relevant to the addiction and relapse processes: self-efficacy, outcome expectancy, attributions of causality, and decision-making processes. Self-efficacy refers to one's ability to deal competently and effectively with high-risk, relapse-provoking situations. Outcome expectancy refer to an individual's expectations about the psychoactive effects of an addictive substance. Attributions of causality refer to an individual's pattern of beliefs that relapse to drug use is a result of internal, or rather external, transient causes (e.g., allowing oneself to make exceptions when faced with what are judged to be unusual circumstances). Finally, decision-making processes are implicated in the relapse process as well. Substance use is the result of multiple decisions whose collective effects result in a consumption of the intoxicant. Furthermore, Marlatt stresses some decisions—referred to as apparently irrelevant decisions—may seem inconsequential to relapse, but may actually have downstream implications that place the user in a high-risk situation.[41]
Different rehabs are based upon different philosophies, and this can mean the enhancement of addiction treatment with distinct specialized services. For example, a religious rehab may include worship services or religion classes and other offerings that enhance the spiritual side of recovery while a holistic alcohol rehab may include yoga, meditation, acupuncture and other alternative methods of treatment.
