The National Institute on Drug Abuse states, “Addiction is defined as a chronic, relapsing brain disease that is characterized by compulsive drug seeking and use, despite harmful consequences.” Addiction can result from a variety of factors and catalysts, including genetic predisposition, circumstances, environment, trauma and mental health disorders. While addiction often starts with drug abuse, it is not an indication of a person’s moral status or stability. In fact, many addictions spring from prescription drug use or casual use of legal substances.
Ecstasy, or MDMA, is classified as both a hallucinogenic drug and a stimulant. Like LSD, PCP, and other psychedelic substances, Ecstasy can alter your sensory perceptions and change the way you perceive time and space. This synthetic drug can also cause feelings of warmth, affection, and intimacy with others — properties that have inspired nicknames like “the Love Drug” and “the Hug Drug.” In addition, Ecstasy is a central nervous system stimulant, increasing energy and activity.
The action stage of change represents full recognition of a problem along with observable evidence of steps taken to reduce alcohol use. The clinician should reinforce and praise the decision to change. Emphasizing that the biggest error at this stage is to underestimate the amount of help needed to quit drinking is critical. The patient should be given a list of options for treatment including AA and pharmacotherapy.
With the Sinclair Method, people only take Revia or Vivitrol before drinking and never otherwise. Revia and Vivitrol are not like other anti-alcohol drugs that cause intense sickness and hangover sensations when taken with alcohol. The change in behavior only appears over time. With the Sinclair Method, Revia or Vivitrol is taken one hour before drinking alcohol. At the end of four to six months of treatment with the Sinclair Method, 80 percent of people who had been overusing alcohol are either drinking moderately or abstaining entirely.
Upon exiting treatment, a patient may be prescribed a drug like disulfiram, which prevents the body from chemically processing alcohol, causing an unpleasant reaction if the patient relapses or attempts to relapse. Because of disulfiram’s toxicity, it has to be taken under the supervision of a doctor, as unregulated usage can cause strong, even fatal reactions.
Focus on one area where you’re experiencing the urge. How do the sensations in that area feel. For example, perhaps you feel hot, cold, tingly, or numb? Are your muscles tense or relaxed? How large an area is involved? Describe the sensations to yourself and any changes that occur. “My mouth feels dry and parched. There is tension in my lips and tongue. I keep swallowing. As I exhale, I can imagine the smell and tingle of a drink.”

In the precontemplation phase, the individual does not express any interest in the need for change. Many individuals with substance use problems are firmly entrenched in this stage. The options in dealing with patients in this stage are limited and include pointing out the discrepancy between patient action and patient goals, suggesting that the patient bring a family member to the next appointment, and suggesting a 2-week abstinence trial. Dependence is defined as an adaptive state that develops in response to repeated drug administration, and is unmasked during withdrawal, which occurs when drug taking stops. Dependence from long-term drug use may have both a somatic component, manifested by physical symptoms, and an emotional–motivation component, manifested by dysphoria. While physical dependence and withdrawal occur with some drugs of abuse (opiates, ethanol), these phenomena are not useful in the diagnosis of addiction because they do not occur with other drugs of abuse (cocaine, amphetamine) and can occur with many drugs that are not abused (propranolol, clonidine). Stimulants, such as tobacco, cocaine or prescription amphetamines, stimulate the brain and nervous system, causing increased alertness. Depressants, such as barbiturates and benzodiazepines, slow activity in the brain and nervous system, causing relaxation.
